Review of medical history, physical exam, immunization review, and required lab work which is a test for syphillis (RPR), gonorrhea (GC), and tuberculosis (QuantiferonGOLD)
If you do not have the required immunizations or vaccines, we can provide them, or blood titers (which is a blood test to demonstrate immunity) for a additional charge
Required immunizations or vaccinations:
2 doses of MMR (measles, mumps, rubella) or blood titer test demonstrating immunity
3 doses of Hepatitis B or blood titer test demonstrating immunity
2 doses or Varicella (Chicken Pox) or blood titer test demonstrating immunity
1 dose of TDAP or td (tetanus) within the past 10 years
Flu shot during Flu season (October-March)
